We ask you to submit to us the form of federal statistical observation No. 1-zdrav Information about the organization providing medical care services to the population (approved by order of Rosstat dated 03.06.2016 No. 266), for 2016. Instructions for completing the form are provided on the form.

Information in the form of federal statistical observation No. 1-zdrav is provided by legal entities, regardless of their departmental subordination (except for micro-enterprises and medical organizations of the Ministry of Health of the Russian Federation) and form of ownership, having a license to carry out medical activities and providing medical care services.

If a legal entity has separate subdivisions, form No. 1-zdrav is filled out both for each separate subdivision and for the legal entity without taking into account these separate subdivisions.

Please note that the definition of a separate subdivision for the purposes of official statistical accounting is provided for by Rosstat Order No. 224 dated April 1, 2014, posted on the Rosstat Internet portal at www.gks.ru. A separate subdivision is understood as any subdivision that is territorially separated from the organization, at the place or from the location of which business activities are carried out at equipped stationary workplaces. A separate subdivision is recognized as such regardless of whether its creation is reflected or not reflected in the constituent or other organizational and administrative documents of the organization, and on the powers vested in the specified subdivision.

The form of federal statistical observation is also provided by branches, representative offices and subdivisions of foreign organizations operating in the territory of the Russian Federation in the manner established for legal entities.

When completing this form, please note the following:

  • in section 1 Legal entities providing medical care services to the population should enter the number 1 only in the box that corresponds to the type of medical organization and its form of ownership. Organizations that have completed line 08 must also enter the number 1 in one of the lines 0801 - 0806, and those who have completed line 09 must also enter the number 1 in line 0901 or 0902;
  • section 2 Structural subdivisions (departments, offices) are filled in by medical organizations of all forms of ownership and departmental affiliation and non-medical organizations that have medical subdivisions in their structure, showing the number of these structural subdivisions.
    Line 13 of section 2 is filled in by organizations that have a medical (paramedical) health center. The technical equipment of the health center should allow to perform the following tasks: providing primary health care, transporting patients and victims to medical and preventive organizations, carrying out a complex of medical and preventive measures to preserve and improve the health of employees of this organization. The functioning of health centers as structural divisions of economic entities is possible provided that medical activities are reflected in their charter;
  • in section 3 The activities of outpatient (inpatient) organizations Line 01 indicates the planned capacity of outpatient organizations (independent and structural units as part of medical and preventive organizations and non-medical organizations that include units of the corresponding profile). If for the premises occupied by the organization its capacity (number of visits per shift) is not indicated in the project, then it is determined as the quotient of dividing the occupied area by the normative indicator of the area, the values ​​​​of which are given on the form No. 1-zdrav in the instructions for filling out. Line 02 of section 3 shows the number of visits to doctors for the reporting year, including visits to dentists. Data are given in units;
  • in section 5 Medical workers information is filled in for the main employees (individuals) without taking into account part-time workers at the end of the year.
  • in section 8 Information on the number of registered diseases reflects data on the number of registered diseases diagnosed for the first time in life.

Medical organizations and non-medical organizations that have subordinate medical units, fill out all sections of the form.

Other organizations with medical staff(educational organizations, recreation organizations, institutions of medical and social expertise, stationary institutions of social services for the population, etc.), fill in only line 09 of section 1, including one of the lines 0901 - 0902, and section 5 medical workers, and in the presence of a medical (paramedical) health center - in section 2, line 13.

Form 1-zdrav "Information on the organization providing medical care to the population", as well as instructions for filling it out, were approved by Order of the Federal State Statistics Service dated 06/19/2013 N 216. The deadline for its submission is January 15 after the reporting period, that is, for 2014 it must be filed on 15.01.2015. In the article, we will consider the features of filling out sections of the form.

This form is filled out by legal entities, regardless of their departmental subordination (except for micro-enterprises and medical organizations of the Ministry of Health) and forms of ownership that have licenses to carry out medical activities and provide medical care to the population.

In the address part, the full name of the reporting organization is indicated in accordance with the constituent documents, its short name is given in brackets.

Note. The medical organizations of the system of the Ministry of Health include organizations created by its order and reporting to the specified ministry.

In the code part of the form, the code of the All-Russian Classifier of Enterprises and Organizations (OKPO) is mandatory, sent (issued) to organizations by the territorial bodies of Rosstat.

This section of the 1-zdrav form is filled out according to the form of ownership on the basis of a notification of the assignment of codes by the territorial bodies of Rosstat and in accordance with the form of ownership (OKFS) of the reporting organization according to its type in accordance with state and municipal healthcare organizations approved by the Ministry of Health.

The number "1" is placed only in the cell that corresponds to the type of medical organization and form of ownership. Line 101 "Reference" is filled in without fail. The reporting legal entity must indicate the type of settlement: urban - the number "1", rural - the number "2".

In this section, institutions show the number of subdivisions, departments or offices in their structure. In particular, outpatient organizations indicate departments (rooms) if the departments (rooms) have the necessary space for placement in accordance with sanitary and hygienic standards and requirements, as well as medical equipment, tools and inventory. In the staff of an outpatient organization, a department is formed, as a rule, with at least four medical positions.

Let us draw your attention to the procedure for filling in line 10 "Department (office) of a general practitioner (family doctor)". It contains the number of general practitioners (family doctors). At the same time, in the instructions for filling out the 1-zdrav form, it is explained that a general practitioner or family doctor is a doctor who has undergone special multidisciplinary training in providing primary health care to family members, regardless of their gender and age. He provides primary health care to a contingent formed on the basis of the free choice of a doctor by patients, forms a medical district from the attached contingent. A general practitioner (family doctor) operates in medical organizations predominantly of the municipal health care system that provide primary health care to the population: in the centers of a general practitioner (family doctor), outpatient clinics and district hospitals of the municipal health system, departments of a general practitioner ( family doctor) outpatient clinics and other medical organizations.

Section 3 is filled in by independent outpatient organizations, including dental organizations, dispensaries that do not have hospitals, medical units without hospitals, health centers, centers (departments) of a general practitioner (family doctor), other institutions providing outpatient care to the population that do not have sick leave beds.

This section contains only two lines. In particular, when filling in line 01, the following features must be taken into account. Planned information is provided.

When a medical organization leases a private healthcare system or another organization providing medical care to the population, from a medical organization of state or municipal ownership of a building (premises) or a bed fund, data on the leased capacity of the organization (number of visits per shift) and hospital beds are reflected only in the reports of institutions - landlords.

When renting a building (premises) from other institutions, organizations, enterprises that are not engaged in the provision of medical services to the population, data on the leased capacity (number of visits per shift) are reflected by the tenant institution in line 01.

If for the building (premises) occupied (own or rented) by the organization, its capacity (number of visits per shift) is not indicated in the project, that is, if the building is adapted, this capacity is determined by calculation:

For organizations and their subdivisions located autonomously (in a separate building, annex, on a separate floor), the power is determined as the quotient of dividing the occupied (working) area by the normative indicator of the area;

For organizations located in a separate complex with other organizations, the area that they directly occupy (working space) is increased at the expense of part of the area of ​​general treatment and diagnostic rooms and auxiliary services, if any, in proportion to the volume of work performed. The total area obtained as a result of such addition is divided by the normative indicator of the area.

CHAPTER 1

GENERAL PROVISIONS

1. State statistical reporting in the form
1-zdrav “Report of an organization providing medical services to the population” (hereinafter referred to as the report) is submitted by legal entities that have a special permit (license) to carry out medical activities, their separate divisions that have a separate balance sheet that provide medical services to the population (hereinafter referred to as organizations) .

The report is submitted by organizations that provide medical services both directly to the population and in accordance with contracts with legal entities for the medical care of their employees.

The report is not submitted:

organizations that provide the population only with the services of solariums (tanning studios) or massage of a non-therapeutic nature;

state institution "Republican Clinical Medical Center" of the Administration of the President of the Republic of Belarus;

state institution "Center for Hygiene and Epidemiology" of the Administration of the President of the Republic of Belarus;

state institution "Republican scientific and practical center of sports";

organizations that are subordinate (included in the system, structure) of the Ministry of Internal Affairs of the Republic of Belarus (including the Department for the Execution of Punishments), the Ministry of Health of the Republic of Belarus, the Ministry of Defense of the Republic of Belarus, the State Security Committee of the Republic of Belarus, the State Border Committee of the Republic of Belarus, the Aviation Department of the Ministry transport and communications of the Republic of Belarus, Ministry of Emergency Situations of the Republic of Belarus.

2. Organizations draw up a report, including data on the subdivisions included in their structure, located on the same territory as them (region, Minsk city).

Organizations whose structure includes subdivisions located in another territory (region, city of Minsk) draw up a separate report for all structural subdivisions located within the same territory, while column 3 of the Respondent Information requisite indicates the code of the territory in which the activities of structural divisions are carried out in accordance with the list in accordance with the appendix to these Instructions.

3. The report is submitted by the organization in the form of an electronic document via the global computer network Internet or on paper.

A necessary condition for submitting a report in the form of an electronic document is that the organization has the means of electronic digital signature received upon registration as a subscriber of the certification center of the republican unitary enterprise "Information and Publishing Center for Taxes and Duties" or a subscriber of the republican certification center of the State system for managing public keys for checking electronic digital signature of the Republic of Belarus of the Republican Unitary Enterprise "National Center for Electronic Services".

The presentation of the report in the form of an electronic document is carried out using specialized software, which is placed along with the necessary instructional materials for its deployment and use on the official website of the National Statistical Committee of the Republic of Belarus in the global computer network Internet http://www. belstat. .

On paper, the organization submits a report to the main statistical office of the region (city of Minsk) at its location by mail or courier.

CHAPTER 2

HOW TO COMPLETE SECTION I

"INFORMATION ABOUT MEDICAL (PHARMACEUTICAL) WORKERS"

4. Section I reflects data on medical (pharmaceutical) employees of the organization, excluding administrative (economists, accountants, and so on) and service personnel (drivers, electricians, and so on) as of December 31 of the reporting year.

5. Table 1 and lines from 075 to 086 of table 2 reflect data only on the main employees of the organization who worked under an employment contract (contract) and performed permanent work in this organization, excluding employees hired part-time from other organizations ( external collaborators).

6. Data on an employee registered in one organization as an internal part-time worker or hired in one organization
for more (less) than one position, in table 2 are reflected as about one person (whole unit) in the main position.

7. Data on persons holding positions of paramedical workers, but who do not have secondary specialized education in the field of education "Health", are not reflected in section I.

8. Table 1 reflects data on employees:

actually showed up for work;

who did not show up for work on the day of rest received for work on public holidays, holidays (non-working) or weekends.

9. In table 1:

data on line 001 must be equal to the sum of data on lines 003 to 053;

data on line 002 must be less than or equal to data on line 001;

line 003 reflects data on chief doctors and their deputies, whose main functions do not include receiving patients;

data on line 054 must be equal to the sum of data on lines 056 to 068;

data on line 055 must be less than or equal to data on line 054;

line 069 reflects data on pharmacists with higher education in the field of education "Healthcare", specialty "Pharmacy";

line 070 reflects data on pharmacists with secondary specialized education in the profile of education "Healthcare", specialty "Pharmacy".

10. In table 2:

for lines 071 and 073, the data is filled in in accordance with the staffing table approved by the head of the organization, for lines 072 and 074 - based on the payroll for December of the reporting year;

data on lines from 071 to 074 are filled with two decimal places;

the sum of the data in lines 075, 077 and 079 must be less than or equal to the data in line 001 of Table 1;

the sum of the data in lines 076, 078 and 080 must be less than or equal to the data in line 002 of table 1;

the sum of the data in lines 081, 083 and 085 must be less than or equal to the data in line 054 of table 1;

the sum of the data in lines 082, 084 and 086 must be less than or equal to the data in line 055 of Table 1.

CHAPTER 3

HOW TO COMPLETE SECTION II

"INFORMATION ABOUT THE ORGANIZATION"

11. Table 3 reflects the presence of specialized departments (offices) available in the organization as of
31 December of the reporting year. An organization that has several specialized departments (offices) of the same name in column 1 of Table 3 indicates the number 1, regardless of the number of these specialized departments (offices). For example, if an organization has three obstetric and gynecological rooms and two pediatric rooms, then in column 1 of table 3 on line 087 and line 089 the number 1 is entered.

The presence of a specialized department (office) is determined by the presence of a separate room, medical equipment, medical devices and positions of relevant specialists in the staffing table. The presence of equipment without the positions provided for by the staffing table for working on it and the allocated premises does not determine the presence of a specialized department (office).

12. Table 4 reflects the number of health posts available in the organization.

13. Table 5 is filled in on the basis of the following forms of primary medical documentation:

A visit should be considered a contact of a person who applied for medical help, advice, a medical opinion or for another reason (for the purpose of undergoing a preventive examination, including the examination of drivers for admission to work, if records are made in the appropriate form of primary medical documentation), with a doctor during the hours provided for by the work schedule for admission to the organization or during the provision of assistance at home.

Table 5 does not reflect the number of visits to doctors working in treatment and auxiliary rooms, that is, performing prescribed procedures and studies (in particular, X-ray diagnostic examinations, X-ray preventive examinations, ultrasound, laboratory, endoscopic examinations).

The data in column 1 must be equal to the sum of the data in columns
2 and 3.

14. Table 6 is filled out on the basis of primary medical documentation  appendices 1, 3 and 4 to the order of the Ministry of Health of the Republic of Belarus dated October 1, 2007 No. 000
"On approval of forms of primary medical documentation in healthcare organizations providing inpatient care":

Columns 1 and 2 reflect data on beds equipped with the necessary equipment and ready to receive patients, regardless of whether they are occupied by patients or not, as of December 31 of the reporting year. The number of beds does not include beds for mothers in children's departments, beds for newborns in maternity wards.

Data on the number of bed-days spent by patients in a hospital are filled in on the basis of Form No. 000/u-07 "Summary record of the movement of patients and bed capacity by hospital, department or bed profile."

Bed data per patient hourly stay is reported as 1 bed-day according to the bed profile.

The data on line 093 in all columns must be equal to the sum of the data on lines 094 to 099 in all columns.

Data on hospital beds rented in other healthcare organizations are not reflected in Table 6.

CHAPTER 4

HOW TO COMPLETE SECTION III

"INFORMATION ON THE NUMBER OF TERMINATIONS OF PREGNANCY (ABORTIONS) UNTIL 22 WEEKS"

15. Section III reflects data on cases of pregnancy terminations (abortions) (hereinafter referred to as abortions) up to 22 weeks.

16. Section III is completed on the basis of the following forms of primary medical documentation:

17. Line 100 shall reflect the total number of abortions performed in the organization, including those initiated and initiated outside the organization, except for abortions performed by the vacuum aspiration method and through the use of drugs.

18. Line 102 shall reflect the number of abortions that ended in death within 42 days after they were performed from causes related to abortion, pregnancy, with the exception of an accident or an accidental cause.

CHAPTER 5

HOW TO COMPLETE SECTION IV

"INFORMATION ON THE NUMBER OF POPULATION DISEASES"

19. Section IV is filled out by organizations whose medical workers not only detect a disease in a patient, but also treat and monitor the patient. Organizations conducting only a consultative reception do not complete section IV.

20. Section IV is completed on the basis of the following forms of primary medical documentation:

Instructions for filling out the form N 1-zdrav

An institution that has in its composition a polyclinic department that conducts only a consultative reception includes information on the incidence only if in this institution the patient is not only diagnosed with a disease, but also the treatment and monitoring of the patient is carried out.

Information on morbidity established by the emergency departments of hospitals in which emergency care is provided in the evening and at night is not included.

Information about diseases detected in patients admitted to the hospital, bypassing the clinic, should be included on a general basis.

The procedure for registering each case, first detected in a hospital, of a disease is determined on the basis of the final diagnoses, information about which is transmitted to the clinic, or taken into account by the clinic when it receives an extract from the inpatient card after the patient leaves the hospital.

Information on the form of federal statistical observation N 1-zdrav is provided by legal entities, regardless of their departmental subordination (except for micro-enterprises and medical institutions of the Ministry of Health and Social Development of the Russian Federation) and forms of ownership that have a license to carry out medical activities and provide medical care services population.

Data are given as of the end of the reporting period (year).

Section 1. Section 2. "Structural units (departments, offices)" is filled in by medical organizations of all forms of ownership and departmental affiliation (with the exception of medical institutions of the Ministry of Health and Social Development of the Russian Federation) and non-medical organizations about the medical units available in their structure, showing the number of these structural units .

Outpatient clinics indicate departments (offices) in cases where the department (office) has the necessary space for placement, in accordance with sanitary and hygienic standards and requirements, as well as medical equipment, tools and inventory. In the staff of an outpatient clinic, a department is organized, as a rule, with at least 4 medical posts.

Line 01 indicates the number of general polyclinics (outpatient clinics), which are structural units within hospital institutions.

Line 02 "Children's polyclinic" indicates the number of children's polyclinics that are part of hospitals or other health facilities.

Line 03 "Children's department (office)" indicates the number of children's departments (rooms) that are part of hospital institutions, and children's departments (offices) of independent general polyclinics.

Line 04 "Women's consultation" indicates the number of women's consultations operating as part of joint institutions.

Line 05 "Obstetrics and gynecology department (room)" indicates the number of obstetrics and gynecology departments (rooms) of polyclinics that are part of hospital institutions and independent polyclinics.

Line 01 indicates the planned capacity of outpatient clinics (APU) - independent and combined with hospitals.

When a medical organization of a private health care system or another institution providing medical care to the population, a building (premises) or a bed fund is rented from a medical institution, data on the leased capacity of the institution (number of visits per shift) and hospital beds are reflected only in the reports of the lessor institutions .

When renting a building (premises) from other institutions, organizations, enterprises that are not engaged in the provision of medical services to the population, data on the leased capacity (number of visits per shift) are reflected by the tenant institution in this report form (line 01).

If for the building (premises) occupied (own or rented) by the institution, its capacity (number of visits per shift) is not indicated in the project, i.e. the building is adapted, then this power is determined by calculation:

For institutions located autonomously (in a separate building, extension, on a separate floor), the capacity is determined as the quotient of dividing the occupied (working) area by the normative indicator of the area;

For institutions located in a separate complex with other institutions, the area that they directly occupy (working space) is increased at the expense of a part of the area of ​​general treatment and diagnostic rooms and auxiliary services, if any, in proportion to the volume of work performed. The total area obtained by such addition is divided by the normative indicator of the area.

The occupied (working) area does not include corridors, vestibules, passages, areas occupied by engineering networks and equipment.

"Information on Termination of Pregnancy" is filled in by organizations in which abortion is performed and includes information on all cases of termination of pregnancy for 27 completed weeks (27 weeks 6 days) as performed in a medical institution (including abortions performed in early terms by vacuum aspiration and medication methods), as well as started or started outside the medical institution, for which the woman was admitted to the hospital.

Of the total number of abortions performed, abortions are distinguished in accordance with the International Statistical Classification of Diseases and Related Health Problems, Tenth Revision: spontaneous, medical, medically indicated. In addition, abortions are shown for social reasons (up to 22 weeks).

The number of medical abortions performed at the request of a woman (up to 12 weeks) includes abortions performed at an early stage using the vacuum aspiration method and the medical method.

Line 04 includes all abortions performed at the request of a woman before 12 weeks of gestation. Of these, line 05 separates early abortions ("mini-abortions"). Taking into account that termination of pregnancy and early terms ("mini-abortions") can be carried out by vacuum aspiration or medical method, line 06 contains abortions performed by medical (pharmaceutical) method, i.e. with the use of medicines (from line 05).

Abortions for medical reasons include abortions performed before 27 completed weeks of pregnancy, when the woman's health is at risk, or based on indications from the fetus, regardless of the method of operation.

Information on the number of reported first-in-life illnesses includes the number of first-in-life illnesses for chronic illnesses, as well as each case of acute illness that may occur more than once during the reporting year (e.g., influenza, intestinal infections, acute myocardial infarction, etc.). Diseases are shown by classes of diseases in accordance with the codes according to the International Statistical Classification of Diseases and Related Health Problems, Tenth Revision (ICD-10).

An institution that has in its composition a polyclinic department that conducts only a consultative reception includes information on the incidence only if in this institution the patient is not only diagnosed with a disease, but also the treatment and monitoring of the patient is carried out.

Information on morbidity established by the emergency departments of hospitals in which emergency care is provided in the evening and at night is not included.

Information about diseases detected in patients admitted to the hospital, bypassing the clinic, should be included on a general basis.

The procedure for registering each case, first detected in a hospital, of a disease is determined on the basis of the final diagnoses, information about which is transmitted to the clinic, or taken into account by the clinic when it receives an extract from the inpatient card after the patient leaves the hospital.
