Pharmacological properties

Pharmacodynamics.

Macropen is a macrolide antibiotic that inhibits the synthesis of proteins in bacterial cells, has a bacteriostatic effect in low doses, and in large doses it is bactericidal. Reversibly binds to the 50S subunit of the bacterial ribosomal membrane. It is effective against intracellular microorganisms: Mycoplasma spp., Chlamydia spp., Legionella spp., Ureaplasma urealyticum; gram-positive bacteria: Stretococcus spp., Staphylococcus spp., Corynebacterium diphtheriae, Listeria monocytogenes, Clostridium spp. and some Gram-negative bacteria: Neisseria spp., Moraxella catarrhalis, Bordetella pertussis, Helicobacter spp., Campylobacter spp., Bacteroides spp.

Pharmacokinetics.

After oral administration, the drug is rapidly and fairly completely absorbed from the gastrointestinal tract (GIT). The maximum concentrations of midecamycin and midecamycin acetate in serum are 0.5-2.5 µg/l and 1.31-3.3 µg/l, respectively, and are reached 1-2 hours after ingestion.

High concentrations of midecamycin and midecamycin acetate are created in the internal organs (especially in the tissues of the lungs, parotid and submandibular glands) and skin. The minimum inhibitory concentration (MIC) persists for 6 hours. The half-life is approximately 1 hour. Communication with proteins - 47% of midecamycin and 3-29% of metabolites.

The drug is metabolized in the liver with the formation of 2 active metabolites with antimicrobial activity. Excreted with bile and to a lesser extent (about 5%) by the kidneys.

With cirrhosis of the liver: plasma concentrations, the area under the concentration-time curve and the half-life increase significantly.

Indications

Infectious and inflammatory diseases caused by microorganisms sensitive to the drug:
  • respiratory tract infections, including those caused by atypical pathogens (mycoplasmas, legionella, chlamydia and Ureaplasma urealyticum): tonsillopharyngitis, acute otitis media, sinusitis, exacerbation of chronic bronchitis, community-acquired pneumonia;
  • urinary tract infections caused by pathogens: mycoplasmas, legionella, chlamydia and Ureaplasma urealyticum;
  • infections of the skin and subcutaneous tissue;
  • for the treatment of enteritis caused by bacteria of the genus Campylobacter,
  • treatment and prevention of diphtheria and whooping cough.

Contraindications

  • Hypersensitivity to midecamycin / midecamycin acetate and any other components of the drug;
  • severe liver failure.

Pregnancy and lactation

The use of the drug during pregnancy is indicated only in cases where the potential benefit to the mother outweighs the risk to the fetus.
Nursing mothers should stop breastfeeding during treatment with Macropen, as the drug is excreted into breast milk.

Dosage and administration

Inside, should be taken before meals.

Children weighing less than 30 kg: the daily dose of midecamycin is 20-40 mg / kg of body weight, divided into 3 doses, or 50 mg / kg of body weight, divided into 2 doses.

The daily dose of midecamycin for severe infections is 50 mg/kg of body weight, divided into 3 doses.

Scheme of appointment for children (daily dose of 50 mg / kg of body weight with 2 times admission):

1 dosing spoon has measured divisions of 1.25 ml, 2.5 ml and 5 ml.

The duration of treatment is usually 7 to 14 days. Chlamydial infections are treated for 14 days.

For the prevention of diphtheria: midecamycin is recommended at a dose of 50 mg / kg / day, divided into 2 doses for 7 days. A control bacteriological examination is recommended after the end of therapy.

In order to prevent whooping cough, midecamycin is recommended at a dose of 50 mg / kg / day for 7-14 days in the first 14 days from the moment of contact.

Suspension preparation:

Add 100 ml of water (boiled or distilled) to the contents of the vial and shake well.

Side effects

From the gastrointestinal tract: loss of appetite, stomatitis, nausea, vomiting and diarrhea, feeling of heaviness in the epigastrium, increased activity of "liver" transaminases and hyperbilirubinemia, jaundice.
In rare cases, severe and prolonged diarrhea may occur, which may indicate the development of pseudomembranous colitis.

Allergic reactions: skin rash, urticaria, pruritus, eosinophilia, bronchospasm.

Other: weakness.

Overdose

There are no reports of cases of serious overdose caused by Macropen.

Possible symptoms: nausea, vomiting. Treatment: symptomatic.

Interaction with other drugs

With the simultaneous administration of ergot alkaloids or carbamazepine with Macropen, their metabolism in the liver decreases and serum concentration increases. Therefore, caution should be exercised when taking these drugs at the same time.
Does not affect the pharmacokinetic parameters of theophylline.
With the simultaneous use of Macropen with cyclosporine or anticoagulants (warfarin), the excretion of the latter slows down.

With long-term therapy, the activity of liver enzymes should be monitored, especially in patients with impaired liver function.
As with any antimicrobial agent, long-term treatment may lead to overgrowth of resistant bacteria. Prolonged diarrhea may indicate the development of pseudomembranous colitis.
The mannitol contained in Macropen (granules for oral suspension) can cause diarrhea.
In the presence of an allergic reaction to acetylsalicylic acid, azo dye E PO (sunset yellow dye, E110) can cause an allergic reaction, up to bronchospasm.

Pharmacological properties

Macropen is a systemic macrolide antibiotic that inhibits protein synthesis in bacterial cells. When using the drug in small doses, a bacteriostatic effect is provided, with an increase in the daily dosage - a pronounced bactericidal effect. According to the instructions for use, the antibiotic is active against such pathogens of infectious diseases:

  • hemophilic bacillus;
  • diphtheria corynobacteria;
  • clostridia;
  • listeria;
  • mycoplasmas;
  • moraxella;
  • staphylococci;
  • streptococci;
  • chlamydia;
  • Helicobacter pylori;
  • ureaplasma.

The active substance of the drug violates the integrity of the membranes of the pathogenic flora, prevents its further reproduction and viability. When administered orally, midecamycin is rapidly absorbed from the digestive tract and enters the bloodstream. The maximum concentration in the blood serum reaches after 1-2 hours, the therapeutic effect is maintained for 6 hours. The metabolic process takes place in the liver. The antibiotic is excreted in the bile, slightly - by the kidneys with urine.

Method of application and dosage

Overdose

There are no specific symptoms of an overdose with a suspension. Most often, a single intake of Macropen in large quantities entails nausea and bouts of vomiting. Instructions for use Macropen (method and dosage)

Take before meals

Adults and children weighing more than 30 kg are prescribed 400 mg (1 tab.) 3 times a day. The maximum daily dose is 1.6 g.

For children weighing less than 30 kg, the daily dose is 20-40 mg per 1 kg of body weight, divided into 3 doses; or 50 mg per 1 kg of body weight, divided into 2 doses. In severe infections, a daily dose of 50 mg per 1 kg of body weight, divided into 3 doses, is recommended.

The suspension is used to prepare a drug for children. The dosage is calculated according to the scheme:

  • up to 5 kg - 3.75 ml (131.25 mg) 2 times a day;
  • up to 10 kg - 7.5 ml (262.5 mg) 2 times a day;
  • up to 15 kg - 10 ml (350 mg) 2 times a day;
  • up to 20 kg - 15 ml (525 mg) 2 times a day;
  • up to 30 kg - 22.5 ml (787.5 mg) 2 times a day.

The course of treatment is 7-14 days, in the treatment of chlamydial infections - 14 days.

  • For the prevention of diphtheria, a dose of 50 mg per 1 kg of body per day, divided into 2 doses, is indicated, the course lasts 7 days. After treatment, a control bacteriological examination is indicated.
  • For the prevention of whooping cough, 50 mg per kg of body weight per day is prescribed for 7-14 days, in the first 14 days from the moment of contact.

To prepare a suspension of granules, 100 ml of boiled or distilled water is added to the contents of the vial. The bottle must be closed and shaken thoroughly. Shake the prepared suspension before use.

Side effects

Macropen sometimes causes the following side effects:

  • nausea, vomiting, diarrhea;
  • feeling of heaviness in the epigastrium;
  • loss of appetite up to the development of anorexia;
  • hyperbilirubinemia;
  • weakness;
  • increased levels of liver enzymes;
  • allergic manifestations on the skin in the form of a rash, urticaria, itching.

Overdose

An overdose of Macropen can cause nausea and vomiting, and no specific treatment is provided. Sorbents should be taken to accelerate the elimination of the drug and symptomatic therapy should be carried out.
