Health food is mandatory method complex treatment sick. The founder of dietology, M. I. Pevzner, believed that nutrition is the background against which others are applied. therapeutic methods treatment, and he developed 15 dietary tables. medical diet for hereditary diseases associated with impaired absorption of substances is the only way treatment, one of the main - with, gastrointestinal diseases . In other cases, compliance with it prevents complications and progression of the disease. A number system of diets has been approved, which is mandatory for all medical and sanatorium institutions, dietary canteens.

Clinical nutrition (diet therapy) is based on the physiology and biochemistry of nutrition, on knowledge of the role of nutrients, a balanced diet and diet. It takes into account the causes, mechanisms and forms of the course of diseases, as well as the peculiarities of digestion in a sick person. Gastroenterology, as the science of treating diseases of the gastrointestinal tract, has a section on the dietary nutrition of patients.

Diet number 5 , Table No. 5 according to Pevzner is recommended for various diseases liver and gallbladder. It is also indicated for chronic colitis with and chronic without pronounced violations. We can say that this is a universal diet, and the most common table, on the basis of which many varieties have been created, which will be discussed below.

Wikipedia offers a characterization of the diet, but a more precise and detailed description of Therapeutic Diet Number 5 can be found in dietetic reference books. They give a description of the main table and its varieties, depending on the stage of the disease (exacerbation, recovery, persistent remission). Varieties of diets, to one degree or another, exclude mechanical and chemical irritants, and also provide for the presence of concomitant diseases.

The 5th dietary table provides for chemical sparing of the liver with good nutrition, as well as improving fat and cholesterol metabolism. In case of liver disease, a sparing diet is indicated, which contributes to the normalization of its function and at the same time improves bile secretion. Strong chemical irritants are essential oils, extractive substances, so they are excluded from the diet. Fried meat, dried and smoked foods are harmful to the liver, have a negative mechanical and chemical effect. Steam or boiled dishes from chopped meat and vegetables have a weak effect.

This diet contains a reduced amount of fats (due to refractory and poorly digestible), salt (6-10 g), limits the use of egg yolks and foods rich in purines . Uric acid in in large numbers found in the liver of animals, the meat of young animals and birds, baker's yeast, smoked sprat, sardines, tuna, sprats, herring, salmon caviar, salmon, dried porcini mushrooms, smoked eel, mackerel, shrimp, mussels. All of them are excluded from the diet - thus, this hypooxalate diet .

It has increased content fiber , pectins and lipotropic substances (therefore it is called lipotropic) - these are essential, found in beef, lean fish. They are rich in soybeans, whey, buttermilk and buckwheat grain. Lipotropic substances protect the liver from fatty degeneration, reduce the risk of cholesterol stones in the bladder and reduce deposition cholesterol in vessels. Fiber has a lipotropic effect, polyunsaturated fatty acid, phytosterols and lecithin . The last three substances are found in vegetable oils (corn, linseed, sunflower and others).

Dishes are cooked stewed, boiled or baked, which provides chemical sparing of the liver. Cooking products by frying is excluded. Dishes are not wiped (only sinewy meat and rough vegetables). It is mandatory to provide fractional nutrition, which contributes to the regular outflow of bile. The calorie content of nutrition is 2400-2600 kcal (proteins - 90 g, carbohydrates - 400 g, fats - 80 g). The use of salt is limited, liquids should be drunk within 1.5 liters.

One of the key products is lean meat and its choice is large - beef, chicken, turkey, veal and even lean pork. It is recommended to eat lean fish, up to 3 times a week. The vegetable composition is also quite diverse: almost all the vegetables we often eat, as well as non-acidic sauerkraut. It is important that fruits and berries are allowed in any form.

List of products that are allowed:

Diet 5th table for cholecystitis

Nutrition for cholecystitis depends on the stage of the disease. In the first days during an exacerbation, in order to maximize the sparing of the gastrointestinal tract, complete starvation is carried out. The patient is only allowed to take liquids: weak tea, diluted juices, rosehip decoctions. Appointed for 3 days Diet No. 5B , excluding any mechanical and chemical irritants. It is recommended for the duration of the patient's stay on strict bed rest (4-5 days).

It limits carbohydrates to 200 g (due to simple ones - sugar, jams), reduces protein (up to 80 g), and the amount of fat. Food is prepared without salt and only pureed. It is important to observe the fractional nutrition (at least 5 times) and take food in small portions. The calorie content of the daily diet is at the level of 1600 kcal, fluid intake is provided (up to 2.5 l / day).

  • Light pureed food with water and no butter.
  • Mucous soups (based on oatmeal, rice and semolina).
  • Liquid pureed porridges (oatmeal and rice) with the addition of milk.
  • Pureed compotes, jelly, vegetable juices.
  • Somewhat later, mashed boiled meat (a little), low-fat cottage cheese, and steam fish are introduced.
  • Wheat bread or crackers.

After 10 days, patients are transferred to for 1-2 weeks. Outside of exacerbation, a moderate sparing of the gallbladder and liver, normalization of the function of bile secretion, which are provided by Table No. 5, is provided. In nutrition, fats are somewhat limited (especially refractory). Dishes are cooked boiled, steamed, and baking without a crust is already allowed. Since nutrition for cholecystitis in remission is aimed at moderate stimulation of bile secretion, it contains:

  • Salads and vinaigrettes seasoned with unrefined vegetable oil (they need to be changed).
  • Various vegetables, berries and fruits.
  • A large amount of fiber (due to the use of cereals, vegetables and fruits), which is necessary in the presence of constipation.
  • Chicken eggs (no more than one), since the yolks are a strong choleretic product. For pain and bitterness in the mouth that may appear when eating eggs, only egg white dishes are allowed.

Diet number 5 can be used up to 1.5-2 years.

Biliary dyskinesia frequent companion gastroduodenitis and . It is characterized by a violation of the tone or motility of the biliary system. Depending on the disorders, dyskinesia can be hypertonic-hyperkinetic ( increased tone and increased motility) and hypotonic-hypokinetic (weak motility and weakened tone of the biliary tract).

Proper nutrition with dyskinesia avoids attacks and is the prevention of cholelithiasis. The diet for these two types contains general rules:

  • Eat small portions (150 grams) and (5-6 times).
  • Observe 3-4 hour intervals.
  • Have dinner 3 hours before bedtime, eating light food (kefir, fruits).
  • Avoid cold or very hot foods.
  • Do not use animal (pork, goose, lamb) or trans fats (margarine, spread).

With hypomotor dyskinesia, foods that stimulate the motility of the gallbladder are allowed: vegetables, fruits, bran, vegetable oil, sour cream, eggs and black bread, cream.

With hypermotor dyskinesia, on the contrary, foods that stimulate bile formation are excluded: fatty dairy products, animal fats, black bread, raw vegetables, broths and sparkling water.

Diet number 5 for cholelithiasis

Nutrition for cholelithiasis during remission does not differ from that described above. In this case, the base table is also shown. With stones in the gallbladder, restrictions apply only to the use of egg yolks - only 0.5 yolks per dish are allowed, indigestible fats are prohibited. Vegetable oils are allowed in an amount that does not cause seizures.

Due to topographic proximity, common blood supply and innervation, with cholelithiasis and cholecystitis, the gastroduodenal system, pancreas (chronic develops) and intestines are involved in the pathological process.

With combined pathology with pancreatitis and cholecystitis, it is used. It is characterized by an increase in protein content (up to 120 g) and an even greater restriction of fats and carbohydrates, which stimulate the function of the pancreas. With cholecystopancreatitis, extractive substances (cabbage broth, meat and fish broths) and coarse fiber of vegetables are necessarily limited. All dishes are served boiled or steamed. The diet is prescribed for 3 months, then it is expanded.

It happens that the underlying disease accompanies gastroduodenitis . Symptoms depend on the stage of the disease, and treatment necessarily includes diet food. With gastroduodenitis in the acute stage, nutrition is prescribed within. Diets have much in common: they exclude dishes - causative agents of gastric secretion. Food is recommended liquid or mushy, boiled and mashed. Exclude the intake of foods rich in fiber (turnips, peas, radishes, beans, radishes, asparagus), fruits with rough skins (gooseberries, dates, currants, grapes), whole grain bread, as well as coarse sinewy meat, poultry skin and fish.

The 5th diet according to Pevzner is often used when in remission, since it provides chemical sparing of the mucosa. At the same time, white cabbage and corn are additionally excluded, as vegetables that have coarse fiber and cause. Barley, corn, barley and millet groats, fatty milk, cream, fermented baked milk are not recommended.

If there is gastritis in the acute stage in combination with cholecystitis, as well as with, especially with severe pain, it is indicated Table №5B . It is considered as sparing as the food is prepared without salt, served only pureed in the form of slimy soups, soufflés and mashed potatoes.

Diet number 5 after gallbladder removal

It should be noted that conservative treatment is not always effective. With frequent exacerbations of cholecystitis, in the presence of complications (purulent, phlegmonous cholecystitis), as well as with cholelithiasis, it is not possible to avoid removal of the gallbladder. Nutrition is the most important component of postoperative recovery period. 12 hours after the operation, it is allowed to drink water without gas in small sips (up to 500 ml per day). On the second day, kefir, unsweetened tea, jelly are introduced into the diet in portions of no more than 0.5 cups with a frequency of 3 hours.

Postoperative diet is extended for 3-4 days - allowed frequent use food (up to 8 times a day) in portions of 150 g: pureed soups on the water, mashed potatoes (semi-liquid), egg white scrambled eggs, grated boiled fish, fruit jelly. You can drink juices (apple, pumpkin) and tea with sugar. On the fifth day, biscuit biscuits and dry wheat bread are introduced. A week later, grated cereals (buckwheat, oatmeal), boiled rolled meat, low-fat cottage cheese and vegetable puree are allowed. The patient can then be transferred to Table No. 5A , a little later - on Table No. 5. Recipes will be given below.

The pathogenesis of the disease is associated with insulin resistance, as a result of which the liver accumulates triglycerides with the formation of fatty hepatosis, which eventually leads to the development of destructive changes in the liver ( steatohepatitis ). In general, the asymptomatic course of the disease is characteristic and it is accidentally detected during a biochemical study and ultrasound. An increase in the activity of ALT, AST and alkaline phosphatase , increase bilirubin , hypercholesterolemia and hypertriglyceridemia . In some patients, it is detected.

In fatty liver disease, weight loss is recommended, which is achieved by diet and physical activity. Patients need to follow the diet of Table No. 5, but in addition, when you need to reduce the energy value of the diet. A minimum calorie intake of at least 1200 kcal for women and 1500 kcal for men is allowed. Weight loss by 5-10% entails a decrease in the activity of ALT, AST, hepatosplenomegaly and helps to reduce steatosis . It is safe to reduce weight by 1500 g per week.

  • Exclusion of butter, margarine, animal fat and the use of foods with polyunsaturated fatty acids (seafood, fish, vegetable oils, poultry, olives, nuts, if the energy requirement allows).

Among the innate functional bilirubinemia , Gilbert's syndrome is in first place. It is believed that this syndrome has 1-5% of the population. The reason is a violation of the binding bilirubin which is inherited. Other pathological changes the liver is not detected. Provoking factors periodic increase bilirubin in the blood are infection, congestion, intake sulfonamides , oral contraceptives , salicylates . In some cases, it first appears during the recovery period with.

The effect of hunger is very pronounced in this disease - an increase in the level of unconjugated bilirubin is noted. Therefore, rational nutrition is necessary within this dietary table and prevention long periods in eating. You also need to observe a sufficient water load to prevent bile thickening and use additional fat-soluble and trace elements.

It is necessary to completely abandon products containing preservatives and dyes, fatty meats and fish. Throughout life, the use of fried foods and alcohol is unacceptable. Nutrition for hepatitis C should be fractional, divided into 5-6 doses.

For kids

In children, functional disorders of the biliary system are more common, less often - inflammatory diseases ( , cholangitis ). However, functional disorders can cause the development of organic pathology of the liver, biliary system and pancreas. bile ducts have a complex system of sphincters and its synchronous activity is extremely important. For dysfunctional disorders, Diet 5 is recommended.

The table for children is no different from that for adults. The basic principles of heat treatment of dishes are preserved. The meal itself is a good stimulant for the flow of bile into the intestines. It follows that meals should be regular. Portions should be given small, avoiding overeating and it is advisable to teach the child to eat at the same time.

Children's diet should be age-appropriate and contain the optimal amount of protein and carbohydrates, and be easily digestible. For a 5-year-old child, the optimal easily digestible protein is the protein of dairy products, eggs and fish (if there is no allergy). In the treatment of dysfunctions with an increase in the tone of the sphincters, the fat content decreases (it is calculated on the basis of 0.5-0.6 g per kg of weight). Fats of vegetable origin are recommended and refractory animal fats are excluded. With hypomotor dyskinesia, the amount of vegetable fats increases to 1.0-1.2 g per kg of the child's weight.

- This treatment table is not prescribed by itself, as a panacea for the disease, but only in combination with medical and physiotherapeutic treatment. It is not recommended to start it on your own - only after consulting a specialist.

With strict adherence to the diet, remission can be achieved quite quickly - eliminate the exacerbation, bring all the digestive organs back to normal. But you will have to follow all the recommendations of the doctor. If it is ordered to eat yesterday's bread, then it is absolutely impossible to eat fresh. If it is said that all roughage is ground, then we do it, otherwise there will simply be no point in the diet.

- A therapeutic diet definitely makes sense in the treatment of such diseases. Oddly enough, nutrition in general can cure many problems in the body. As for table 5a, it allows not only to improve general state and stop the exacerbation, but also reduce weight, raise the overall tone.

Several useful tips: prohibited foods should not be consumed during and after the diet. Forget about alcohol, alcohol and delicious smoked meats with spicy spices. Otherwise, the whole result of the diet will go down the drain. Loads on internal organs should not be at all - only in this case it will be possible to normalize their work. The course of the diet, if necessary, is repeated, but only on the recommendation of a doctor.

- With cholecystitis and chronic hepatitis it is extremely important to follow the key principles of the therapeutic diet.

I decided to support my friend, she had to stick to this diet because of liver problems, and I didn’t want her to be bored and lonely) but I didn’t think that I would be so dragged out, even after the end of the diet I try to stick to it so that the result achieved lasts longer!

I have had dyskinesia since childhood. Sometimes, with improper nutrition, there are exacerbations, so you have to use this diet (although for good, always stick to it) and, fortunately, the pain and all the unpleasant symptoms go away. So get well and eat right, limiting prohibited foods and there will be no exacerbations!

A couple of years ago I had serious problems with cookies. She was treated for a long time, and during the recovery period, the doctor prescribed this diet. What can I say, it was rather unusual, since I had to meticulously follow all the rules of the treatment table. A very important role is played by the products in the diet, and the way they are prepared, and the time of consumption. Never before have we paid so much attention to the food we eat. But what can you do ... It's better not to joke with health.
Now, by the way, I can already eat the usual foods, but the diet gave me a good hardening 🙂 So I still mechanically adhere to the principles of diet No. 5 and am very glad about it) I lost weight, I feel great!
