Another important nuance, which should not be forgotten - the validity of the sanatorium-resort card is only two months. Do not delay the medical examination and take into account the factors that may interfere with you, i.e. you need to go for the coveted form no earlier than a couple of months in advance. The deadline for obtaining a certificate is 10 days before the trip, otherwise you risk being left at home. There is also quick way make a document, but it’s better not to risk it and go through the examination yourself. Don't lose the return coupon you receive. It must be attached to the outpatient card.

In order to improve their overall health, children are often sent to sanatoriums. A voucher for a child can be issued on the recommendation of a local therapist in the presence of chronic diseases. The main thing is that the disease is not in acute stage at the time of the trip, and the diagnosis was accurate. If a minor who is registered goes on vacation, a sanatorium-resort card for children is filled out by a specific specialist.

A certificate for obtaining a voucher (form 070/у-04) is a medical document that is preliminary, informational in nature and represents a list medical recommendations for getting sanatorium voucher.

A certificate for obtaining a voucher (form 070/u-04) is a medical document that is of a preliminary informational nature and is a list of medical recommendations for obtaining a sanatorium voucher. The Federal Law “On the procedure for medical recruitment and referral of patients for sanatorium-resort treatment” (Article 256 of November 22, 2004) states that a certificate for obtaining a voucher does not replace a sanatorium-resort card and does not give the right to enter sanatorium-resort organizations for spa treatment, which can also be provided on an outpatient basis. In other words, a certificate for obtaining a voucher indicates that the patient needs sanatorium treatment, but does not give any rights to receive a sanatorium voucher.

The purpose of the certificate for obtaining a voucher (form 070/u-04) may be a little lost in this formulation of the law and may not be entirely clear to a patient in need of sanatorium-resort treatment. Let’s try to find out: “Why do you need a certificate to get a permit?”

Let's start with the fact that any trips to a sanatorium are always paid. Almost all sanatoriums are self-supporting organizations and someone must pay for the accommodation and treatment of patients. The whole question is: “Who will pay?” Often, for months or even years, participants in the Great Patriotic War (WWII), combat veterans, disabled people and other beneficiaries wait for a free sanatorium voucher. They wait a long time, not because there are no places in the sanatoriums, but because they wait until the Fund social insurance will allocate funds to pay for this sanatorium voucher. Sometimes, large and wealthy enterprises can assume the functions of social protection for their employees, paying them in whole or in part for sanatorium-resort treatment from internal funds or at the expense of the trade union. A trip to the sanatorium can also be purchased privately with 100% payment of the cost.

So why do you need a medical certificate to get a permit? Here's what it's for! In order for a patient not to receive a ticket to a sanatorium, the profile of which does not correspond to his diseases, and in order to be sure that the sanatorium is not located in an unsuitable natural and climatic zone, it is necessary to know what diseases the patient suffers from, what is indicated for him and what is contraindicated! Those. certificate form 070/u-04 (certificate for obtaining a voucher) will accurately inform everyone involved in the allocation of sanatorium vouchers, which sanatorium voucher is suitable and which is not suitable for a particular patient in need of sanatorium-resort treatment.

Usually, medical certificate Form 070/u-04 is issued by the medical treatment and preventive commission of the local clinic represented by the local general practitioner. This certificate must be provided to social security services (SOBES), or at the place of work, or presented to the agency in person (if independent purchase vouchers to the sanatorium). Although, when purchasing a trip to a sanatorium on your own, you may not be asked for a certificate to obtain a trip, it is still better to provide it, so that later it does not turn out that you will pay money for a trip to a sanatorium, where you will be contraindicated.

The validity period of a medical certificate form 070/u-04 for obtaining a voucher is six months from the date of issue. The certificate for obtaining a voucher must contain accurate information on the profile group of the sanatorium and the time of year (season) favorable for treatment. The main and accompanying diagnoses must be coded in accordance with the International Classification of Diseases (ICD-10). Before purchasing a trip to a sanatorium, check to see if you belong to the category of citizens to whom sanatorium-resort treatment should be provided on preferential terms (free of charge or with partial payment of the cost).

According to Article 125 federal law(dated August 22, 2004), the right to preferential receipt The following categories of citizens have vouchers to the sanatorium:

1) participants, disabled people and veterans of the Great Patriotic War;

2) people, medal recipients"Resident of besieged Leningrad";

3) people who worked in the rear during the Great Patriotic War;

4) family members of participants, disabled people and veterans of the Great Patriotic War and family members of deceased hospital workers in besieged Leningrad;

5) disabled people;

6) disabled children.
